To the Editor I read with great interest the updated analysis of the SONIA trial by Wortelboer et al, which demonstrated that first-line use of cyclin-dependent kinase 4 and 6 inhibitors (CDK4/6i) did not improve overall survival (OS) compared with second-line use in hormone receptor–positive advanced breast cancer. This trial provides pivotal data challenging the paradigm of moving targeted therapies to earlier courses.
