Background Active travel (AT) is positively associated with human and planetary health, but little is known about the differential impacts of AT interventions on population groups subject to inequity. We aimed to produce a systematic map to describe the extent and nature of evidence assessing equity-related impacts of AT interventions. Methods We searched 31 databases to identify systematic reviews and primary research evaluations of AT interventions with quantitative data on active travel behaviours (e.g. walking, cycling) and/or health in populations disadvantaged by age, sex/gender, ethnicity, socio-economic status (SES), disability, or sexuality. Following title-abstract screening by one reviewer, two reviewers independently screened full-text articles and extracted information concerning population, intervention, outcome, setting and study design characteristics. Results We included 150 studies: 27 systematic reviews (SRs) and 123 primary studies. They were predominantly concerned with active school travel (AST) (85% of SRs; 69% of primary studies), with our primary focus being on the non-AST evidence, derived from 4 SRs and 38 primary studies. Of these 4 SRs, one focused exclusively on older adults, and three conducted subgroup analyses (two by ethnicity and SES, one by sex/gender). Of the 38 non-AST primary studies, 15 focused exclusively on older adults, ethnic minorities and/or low SES populations (of which 7 conducted additional subgroup analyses by age, sex/gender and/or SES). The remaining 23 studies focused on general populations but conducted subgroup analyses by age (n=12), sex/gender (n=19), ethnicity (n=6), SES (n=14) and disability (n=1). Most used longitudinal or quasi-experimental designs; there were five randomised controlled trials. Conclusions To date, research on differential impacts of AT interventions has focused predominantly on active school travel. Data on impacts for other populations and dimensions of equity remain sparse, particularly regarding ethnicity and disability. These evidence gaps merit further investigation.
