These can be difficult, but the key is finding the right beginning and ending points for the lane components. Here's the breakdown:
Before the on-ramp, you'll have three lane components on the main roadway.
Add a fourth lane component that starts in the intersection for the on-ramp. It will be trial-and-error to find the correct place to start it, but when it is done properly, the intersection object will handle the transition nicely.
End the third and fourth lane components in the intersection for the off-ramp. Again, it will be trial-and-error to find the right spot, but then the intersection will do the transitioning. It looks like you were trying to use the "transition out" toggles for the lane components. Turn those off and just have an abrupt end.
After the off-ramp, there will only be two lane components on the main roadway.
