Recurrent Glaucoma Progression—Cupping Begets Cupping

Malek et al from the Bascom Palmer Eye Institute at the University of Miami have conducted a retrospective cohort study of patients with suspected glaucoma and open-angle glaucoma to identify risk factors for recurrent visual field progression following an initial progression event. Progression was determined using the Guided Progression Analysis software available on the Humphrey Field Analyzer (Carl Zeiss Meditec). During a mean follow-up of more than 12 years, 1951 of 7457 eyes (26.2%) had an initial progression event; among the 635 eyes with adequate follow-up after first progression and intraocular pressure (IOP) data, 167 (26.3%) went on to manifest a second progression event (13.8% within 3 years; 43.5% within 10 years). Risk factors for recurrent progression included faster decline in mean deviation (MD) preceding the initial progression, high mean IOP following initial progression, less negative MD at initial progression, and older age.
