Seeking Nirvana in Academic Surgery—Where the Grass Is Greener

Well-being and professional burnout concerns are not new in surgery. Investigators have attempted to identify causes and potential interventions to improve global wellness among surgeons. In a 2009 cross-sectional survey of the American College of Surgeons’ membership, Shanafelt et al found a significant percentage of respondents expressing feelings of emotional exhaustion (53%), depersonalization (47%), and symptoms of depression (30%). While those findings highlighted the stress of a surgical career, an overwhelming majority of respondents said they would choose their specialty again. Over a decade later, a systematic literature review did not demonstrate any evidence of rising rates of surgeon burnout in the US and Canada. Further analysis showed that rates of emotional exhaustion and depersonalization had decreased over the decades studied. A 2026 survey of all physicians found no decline in happiness, work-life balance, or family relationships compared with previous surveys. However, a disheartening finding in this survey was that general surgeons scored the lowest among respondents when assessing the odds of balancing personal happiness with their careers as only 60% felt they had achieved a balance. The sobering message in these studies is important and academic faculty are not the exception. Sarkissyan et al studied retention from the perspective of academic surgical faculty and found that nearly one-half of surveyed faculty were seriously considering leaving their current jobs citing discontent with governance, lack of work-life balance, and feeling burned out as the top 3 drivers. There is hope, however, as Mahoney et al looked at job satisfaction of private practice and academic surgeons and their results showed that 85% of academic surgeons were satisfied with their career.
