In Reply We appreciate the thoughtful commentaries by Li et al and Xiong and Yan regarding our Prophylactic Treatment of the Ductus Arteriosus in Preterm Infants by Acetaminophen (TREOCAPA) trial. Both letters highlight the paradox of our findings—prophylactic acetaminophen significantly accelerated ductal closure yet did not improve survival without severe morbidity. We agree that the dissociation between anatomical closure and clinical outcomes is the most consequential finding of TREOCAPA, confirming a consistent pattern across pharmacological patent ductus arteriosus (PDA) trials regardless of the agent used.
