Reassessing Treatment Outcomes Following Immune-Related Adverse Events

To the Editor The article by Block et al suggests an immediate dramatic effect of dupilumab on mortality following a cutaneous immune-related adverse event. They found that within 200 days of initiation, approximately 17% of patients given systemic corticosteroids died whereas 3% of those given dupilumab died (a risk difference of 14%). Such an immediate separation of survival curves in an observational study of cutaneous drug reactions strongly suggests unmeasured confounding, which is likely, given the inability to adjust for oncologic variables and year of treatment.
