In the end, it took a child, untouched by those pressures, to say plainly that the emperor was in fact naked. Prostate cancer care is unfortunately highly vulnerable to the same error. Key opinion leaders, professional societies, reimbursement incentives, and market competition all influence clinicians, and new technologies arrive from industry dressed in the language of precision, personalization, and reduced morbidity. Two studies in this issue of JAMA ask that care be judged by its evidence rather than its appearance.
