This is just the disk. I kind of doubt there was ever any printed
documentation. I found an ad showing that Rainbow Computing
sold it for $19.95 in 1979.
It is a little bit shocking that this was for sale. The program is
more like a sample program in a manual. It boots on a 13-sector,
Applesoft ROM Apple II. It loads a shape table from cassette, and
then allows you to plot the shapes on a screen with axes printed on
it. No rotation, just scaling. Your options are to plot another
shape or clear the screen and start over.
I can't imagine anyone ending up excited to finally have received their
Shape Scaler disk in the mail.
