The treatment of early-stage human epidermal growth factor receptor 2 (ERBB2; previously HER2)–positive breast cancer has changed profoundly over the past 3 decades. A disease once defined by aggressive behavior is now often curable, largely because ERBB2-directed therapy has been successfully combined with chemotherapy. This progress has changed the question that clinicians and patients face. For many patients, the issue is no longer only how much treatment is required to prevent recurrence but how much treatment can be safely avoided.
